Ottehr – modern, modular electronic health record platform

Ottehr is a modern, modular electronic health record platform intended as a foundation for building and white-labeling production healthcare applications.

It’s designed around a monorepo containing patient-facing, staff-facing, and backend components, and uses Oystehr as the hosted backend service provider when run as supplied. The project is written mainly in TypeScript and is aimed at developers creating custom EHRs, patient intake systems, telemedicine workflows, and other health-tech products.

This is free-to-download, source-available software, but its license is not open source-compatible.

Key Features

  • Provides a patient portal for appointment registration, rescheduling, check-in, intake paperwork, messaging, and telemedicine call initiation.
  • Includes a staff-facing EHR for managing appointments, patient queues, encounters, charting, eRx, assessments, care plans, and claims workflows.
  • Supplies backend Function-as-a-Service endpoints for the patient portal and EHR applications.
  • Uses a monorepo structure with separate frontend apps, backend packages, shared utilities, and UI components.
  • Supports local development with Node.js, npm scripts, and environment-based configuration.
  • Offers customization and theming options for adapting the application name, branding, imagery, colors, website details, and support details.
  • Includes end-to-end testing documentation and scripts for validating intake and EHR workflows.
