Cacti
Cacti is an open source, web-based frontend to
RRDtool, the industry standard, high performance data
logging and graphing system for time series data.
Cacti stores all of the necessary information to create graphs
and populates them with data in a MySQL
database. The frontend is completely PHP driven. Along with being able
to maintain graphs, data sources, and round robin archives in a
database, Cacti handles the data gathering.
There is also SNMP
support for those used to creating traffic graphs with MRTG.

graph optionally utilizing CDEFs or data sources from within Cacti
- Automatic grouping of GPRINT graph items to AREA, STACK,
and LINE[1-3] to allow for quick re-sequencing of graph items
